We need to have a single master ToC for the entire book in order to present a consistent navigation experience a la readthedocs. This hides the ToCs included at the bottom of many individual pages and adds a ToC that includes ALL pages in the repo.

@rose-coste We can't :shipit: the user guides until this is merged.
