9 years ago https://github.com/rackerlabs/docs-core-infra-user-guide/pull/313 fixes this so that everything within /cloud-images-product-concepts/sharing-images/ shows in the TOC.
Hidden pages (available by previous/next but not shown in the TOC) are caused by subsections in the index.rst for the directory. If that index.rst has subsections then they are listed in the TOC but the rest of the toctree is not. Pulling that index.rst's subsections into separate pages, mentioned in the toctree along with the rest of the directory, lets everything in the toctree be shown in the TOC.
I'm checking the rest of the guide next for other occurences of this, so not closing the issue yet.
for example, http://rackspace-core-infra-user-guide.readthedocs.org/en/latest/cloud-config/compute/cloud-images-product-concepts/sharing-images/models.html "Models of sharing images" does not appear in the left-side TOC; it's only available through previous/next navigation:
