CLB node status changes #642

Closes #629.

Newly created CLB node's status will be OFFLINE if corresponding CLB's health monitor is disabled.
Control plane API is implemented to update CLB node status.

If you can address the documentation issues above I think this is good to merge.

Just running otter integration tests in https://github.com/rackerlabs/otter/pull/1898 to make sure it doesn't cause any regression.
