Release to Supermarket #16

With many customer cookbooks and our templates starting to use this, could we release it to Supermarket? Is it time to :shipit:?

Once https://github.com/rackspace-cookbooks/rackspace_cloud_monitoring/pull/18 is merged, I'll do the release :-)

This is mostly completed, but we need to add rackops as a collaborator.
