Document all plugins #120

In https://github.com/ractivejs/ractive/pull/3052, the packages will have a blank readme (or at least a link to their docs page). Documentation of the official plugins should be written in the docs site instead. This is so that there's only a single source of truth.

https://ractive.js.org/integrations/ will be the place where it will all happen and documented like the Plugins page, by type and one after the other.
