Could not add a new instance of mplayer #50

What steps will reproduce the problem?
1.in moodle 1.9 with latest mplayer. add activity to course topic
2.choose flv file (leave all advanced options alone)
3. r-video> Could not add a new instance of mplayer

What is the expected output? What do you see instead?
Expect to see mplayer with video. Currently have several older videos that work 
where we used mplayer module several months ago. Just get error msg that it 
could not create instance of mplayer.

What version of the product are you using? On what operating system?
Moodle 1.9 on Windows based server using MySQL

Please provide any additional information below.
