exercises table:

new colum paste_visibility, nullable: true using rails enum feature (https://dev.to/ousmanedev/use-activerecord-enum-for-your-enumerations-p7k)
- values from course's visibility policy (https://github.com/testmycode/tmc-server/blob/bs4/app/controllers/submissions_controller.rb#L321-L333)

How the paste visibility policy should work on the submissions controller:

If exercise has a paste visibility policy use that
If exercise paste visibility policy is null, use the policy from course
