Add Heroku button, update docs #39

I don't think this will work, as we need to create a Fourchette app. What I think I would do is create a new app, and make the Heroku Button here use that repo (there is a param for this).

Follow the steps to setup a new app, commit, push to a new repo. Have that repo be the source of the Heroku button. The app.json should be there too as far as I understand.

At least, that's my current theory :)
