Added GitHub issue and pull request templates. #25

Overview

Provides templates that issue or pull request authors can use as a guiding standard for the project. This will help ensure some consistency for project maintainers.

For me, personally, this is the same standard I use for auto-generated Ruby gems as provided by the Gemsmith gem.

Notes

These are the templates I like to use but don't have any issues with making corrections based on what the project maintainers would prefer. Whatever the direction, it's good to be consistent. :smile:

This is a good idea! Since this is just a license so there's no debugging/steps to reproduce, maybe we adjust the headings to:

Yeah, that works for me. Thanks. I've amended the original commit with the changes. Regarding PRs:

I opted to simplify it by using Notes only. Otherwise, everything is changed to what is described above.
