Add server-side examples for codeigniter #44

This isn't directly related to CodeIgniter, but I was about to request the addition of a package for Laravel that would require adding a new heading for PHP. Since this adds that heading, maybe it would be easier to piggyback on this PR:

Laravel HTMX should probably be listed under the "Tools" section - feel free to send a PR.

In this PR I added the server-side example that contains a real app built with HTMX and CodeIgniter.
