In some configurations (newworld ROM?) a sound output device is available that works properly when used, but it is not selected by default as the sound output device within the guest Mac OS.
The Mac OS 8.5/8.6 version of the Monitors & Sound control panel, what is provided for sound settings in those versions, doesn't actually have the functionality of selecting the sound output device, you have to use the old Sound control panel to set it on those versions (though that is provided in Apple Extras on those versions)
