When moving the cursor down through pull-down menus I notice some glitchyness of the selection drawing, where it appears in multiple separate horizontal blocks, suggestive of screen tearing.
I know in the internals somewhere there is some kind of frame interval. I don't know if that's hooked up to anything.
Look into what vsync we can get basically for free on the output side from SDL2, and look into hooking it up to the mac side.
