Provide R function to trigger edge and therapy episode (re)construction

[x] for SQLiteConnection
[x] fix incorrect behaviour currently .load_prescriptions.SQLiteConnection() does NOT necessarily retrigger .create_table_therapy_episodes if combination/therapy ids exist in the source data. .create_table_therapy_episodes should always be triggered even if the edge table doesn't need building.
