Build a model to handle system-specific handling of TTA prescriptions

[ ] split the TTA out of a regular prescription at discharge for systems that do handle discharge medications explicitly
[ ] mark whether dispensed or not
[ ] build report
[ ] currently therapy episodes do not span prescription_context. This may need to change. Users will want to choose to compute LOT both inpatient and inpatient + TTA
