Unable to view user action history after uploading audio to enquiry #226

Steps to reproduce:

Create a new enquiry on the web
Edit the enquiry to add audio
Navigate to My Account >> User action history

Expected Result The log of all the user's actions, including adding an audio file to an enquiry record should be displayed

Actual Result Throws error. See screenshot below:

This behaviour is not exclusive to uploading audio. This is also occurring when photos are added to either a child or enquiry record. A slight variation of the error thrown for audio is shown in the screenshot below:
