Screen 1.1.0 User type drop down option #58

This screen is the very first screen when user clicks on "create account".This has drop down option for user to create account based on whether if: 1.Patient is 18 or above 2.Caregiver representing a patient under 18 3.Caregiver representing a patient 18 or above but with special needs. Based on the choice,the credentials are collected in the Database accordingly
