"Default Int. Prefix" code can cause doubling up of the prefix #654

What steps will reproduce the problem?
1. Enter a "Default Int. Prefix" in WebSMS settings (e.g. "+61").
2. Send a message to a number without 00 or a '+' sign (e.g. "61xxxxxxxxx").
3. WebSMS will add the default prefix even though it is already there (e.g. it 
will try to send to "6161xxxxxxxxx").

It's possible that this issue only exists when FishText is used (that's the 
only connector I use), but it seems unlikely considering the "Default Int. 
Prefix" settings are part of the general WebSMS settings.

I'm running WebSMS 4.2.7, and, not that it matters, CM9 ICS 4.0.3.
