Migrate to IdentityServer4 v. 4.0.4 #4

fixes #3

upgrade libraries
synchronize sample application with identity server repo version
add CorsPolicyService
use inheritance for Resources like in IDS
use single project for package, i.e. bring extension methods to main project
use new package id RavenDB.Contrib.IdentityServer4

This is the first iteration and I'll be testing this now and possibly updating.
