Update readme to target .NET 4.5 and Raven 2.5 #29

Unless we still want to be compatible with 4.0.

For now, 2.5 stuff has it's own branch. I'll probably switch these at some point, putting the current 2.0 stuff in a separate branch so there's at least one published version of Raven.Client.Contrib for 2.0 on nuget.
