Few tasks:

The headers should be:

MONTH,FILE_NAME,HARD_CODE_SECR,SUSP_COMM,MD5_USAG,HTTP_USAG,BIND_USAG,EMPT_PASS,DFLT_ADMN,HARD_CODE_UNAME,HARD_CODE_PASS,MISS_DFLT,INTE_CHCK,TOTAL
Need full path to file e.g. /Users/akond/SECU_REPOS/test-chef/consul_lwrp-2016-12/recipes/default.rb
Need month info, for test examples use 2019-03

The output CSV file is missing the following fields: 'HARD_CODE_UNAME' and 'HARD_CODE_PASS'
