Implement RNG annotations for capturing information required by DITA coding requirements #1

Need XML markup for RNG annotations to provide the details required by both the 
DITA coding requirements as well as the TC's conventions for comment content in 
TC-defined modules and document type shells.

I have committed a first-stab RNG schema to 
org.dita-ng.doctypes/rng/rngDitaAnnotations. 

I'm not sure how to use this module from RNG documents in order to get 
validation/editing support for the annotations within Oxygen.
