Add local/remote port, address to Source Map for DICOM Listener

Add the port and address variables to the source map when using the DICOM listener, exactly as we have now for the TCP Listener. Currently, there is no way to get address/port of the sender with DICOM.

Revision 8224: Added more information to the source map for DICOM Listeners. Variables for AETs, socket addresses/ports, AC/RQ contexts, presentation contexts, user principal, and TLS information were added.

The following source map variables were added:

• localApplicationEntityTitle
• remoteApplicationEntityTitle
• localAddress
• localPort
• If the remote socket address is an InetSocketAddress: remoteAddress remotePort
• If an Associate-AC was found: associateACProtocolVersion associateACImplClassUID associateACImplVersionName associateACApplicationContext associateACPresentationContexts If there is at least one PC: ** *associateRQProtocolVersion**
• If an Associate-RQ was found: associateRQImplClassUID associateRQImplVersionName associateRQApplicationContext associateRQPresentationContexts If there is at least one PC: ** *associateRQPresentationContexts If a user identity is present: * username ** *passcode * userIdentityType
