Mirth omitting some fields from DICOM response #4025

Mirth is omitting AffectedSOPClassUID and AffectedSOPInstanceUID from certain operation responses (C-ECHO, C-STORE). this causes incompatibility errors with at least one software package (pynetdicom)

it appears that the standard violation is with dcm4chee as http://dicom.nema.org/medical/Dicom/2016a/output/chtml/part07/sect_9.3.5.2.html mandates AffectedSOPUIDClass in C-ECHO-RSP
