install ruby 2.0.0-p247
ln -s rbenv/versions/2.0.0-p247 rbenv/versions/2.0.0
run something -> 247 is cached
install ruby 2.0.0-p353
ln -sf rbenv/versions/2.0.0-p353 rbenv/versions/2.0.0
go into directory with .ruby-version 2.0.0 -> gem env includes both ruby version gem paths

It's a pretty abstract edge-case, but maybe the plugin could not leave the modified GEM_PATH around it adds ...

Are you using the latest git revision of gem-rehash? There is no caching mechanism anymore
