SDS Chart

This is a large refactor and includes a jump in the major version to 4.0 It will only work with version 6.0 of the charting package

• adds new props (including cut and paste)
• new style props (for the sds charts)
• restyling of the form and buttons
• move the midparental height into the main form and lose the utilities tab

This completes V1 and minimum viable product
