Changes to lm_thread_t #126

Follow up of #84

Possible changes:

Add lm_pid_t owner so that it no longer needs a LM_GetThreadProcess function. Instead, LM_GetProcessEx could be used
Add lm_time_t start_time to identify the thread start time, which could be useful for finding the first created thread in the process, and also to know if the thread didn't die
