changelog: removed duplicates and fixed inconsistent formatting #177

There were apparently some merging issues that resulted in duplicate changelog entries; this PR removes the duplicates. It also makes a couple minor consistency changes to changelog item descriptions.

Note that this definitely isn't release-blocking! I can change this to point at master if the changelog gets merged before this.
