Resurrecting #314. Add --source-host option to choose between GitHub, GitLab, Bitbucket for:
project link in titlebar (--source-host-url)
'Show on Xxxxx' per-declaration links (--source-host-files-url)
Backwards compatible, the options and mustache tags involving "github" are aliased onto the new repo-agnostic terms.
Fix bugs with the per-declaration links found along the way.

The GitLab icon. I've used their (official) mono version here because the jazzy color scheme in the header is basically b+w. Their more familiar colourful one pops 'looks great' in that it pops really strongly and looks like the most important thing on the page!
It's easy to replace the image using a custom theme, minded to go ahead with the mono version.
