Add an issue template #1

There's a number of things that are useful in onboarding a public dataset to ADX that we can ask for in the issue template such as link to the dataset, link to the license, etc.

It looks like the issue template can't by default add it to the project, I think we can make one of us the default assignee to make sure we do it, in addition to asking them to do it in the docs.
