Auto generate the channel inclusion file when calling mc2xml #309

Users can create their own channel include file per lineup and it will be used, 
but why not create it automatically based on the disabled channels in SageTV?  
If the user doesn't have one, create one automatically and use it.  This will 
also reduce the size of the xml file.

Doing this would cause new channels to a lineup not to be seen/added because 
the downloaded xml file from mc2xml only includes the channel info for channels 
included.  mc2xml either needs to support a list of channels to NOT include 
and/or it needs to download and include the channel info for ALL channels on a 
lineup even if the listings for those channels is not being downloaded.
