The "Okay" and "Cancel" and "Apply changes" buttons should be bottom right in relationship to their subject. Right now they're generally top left, up by the menu items, near the top. This makes no sense to me. Since the approval or cancellation happens logically and temporally after what we're approving, they should appear after them in reading order. I hate to say it, but Adobe/Microsoft got this right.
