Allow users to edit the profile with JavaScript disabled #131

Some people are having trouble here on logincidadao.rs.gov.br. We believe that's because they are either with JavaScript disabled or with a connection slow enough to allow 'em to edit the profile and try to post it before the JS fully loads.

With this changes we degrade gracefully for these people by not showing the place of birth fields until the necessary JS loads.
