Assessment Notes not visible #115

As an assessor I can add free text notes at the end of an application assessment. Once the assessment as been completed, the notes are not visible anywhere.

assessor notes are now visible on the view and review assessment pages above the question results and ratings. I also added the app description if one was provided.
