Worked with ccopello, this PR contains work that he did on his PR for Issue #4 Directory Structure (which can probably be closed now) Also addresses Issue #22 All headings must have IDs for the structure.adoc file.

Also, @adahms , do we want to split out the second half of this file as a "production" or "publication" file since everything after the "rendering" topic isn't really about structure anymore?

This looks great! I think we might want to link out to the modular docs public repo when talking about breaking content into pieces, but merging this because it's great!
