Converted the content on migrating an existing repository into separate files. #95

@ncbaratta @pmkovar - This is the follow up to my original pull request to submit this content. In this pull request, I have split the content into separate files, and have edited the content a little so that it more generally applies to a combined downstream and upstream audience.

@pmkovar - I have now submitted a new commit that adds the new chapter to the community version of the guide, and fixes the typo. Thank you for the review!
