Remove "// Module included in the following assemblies section" #154

Removed instances of "// Module included in the following assemblies" from the manual and templates. Resolves issue #149

@rolfedh, I've just noticed that the assembly template also has the "included in" header but you didn't remove it there. Do you want to keep it or did you overlook it?

Thanks, @mrksu. I overlooked it and have removed it with https://github.com/redhat-documentation/modular-docs/pull/156.
