Create Open Science Success Story Solicitation Feature and Database #1

We love hearing about how open science has improved people's lives! However, we currently do not have the infrastructure set up to ingest and store that data in a clean and efficient way. Your task will be to devise and implement a frontend method of soliciting and a backend method of storing those success stories, as well as making those stories publically available and searchable. This could be as simple as a Google Form, but of course, the more integrated this feature is with the Github repo the better. One potential inspiration is the Open Science Success Story Database.
