Upgrade "Issue credits" admin interface with additional fields

These additional fields should be:

vintage period;
credit class version;
methodology version;
verifier: select from existing organization or create new one with org name, user (org's owner) name, email and role.
metadata (in particular for buffer pool allocation or other metadata related to initial issuance on 3rd party program if relevant)
