fix: ignore highlighting for no lang #140

No lang in the meta and no default code lang (defaults to falsy value "") means all highlighting is skipped entirely, and the <pre><code> tags and text content are raw.

Specifying plaintext as a default lang ensures all code blocks will get default highlighting like before.
