Changes in this PR:

removes bracketed source information from the gene_description column
moves position of gene_description column (out of implementation convenience)
adds description source info to two new columns: gene_symbol_source_db and gene_symbol_source_id
renames the gene_symbol_source column to gene_symbol_source_id for consistency
