headphones-modified folder created in each download

So I turned off all post processing and I'm not using torrents (and the seed checkbox is off) yet in each download I get a headphones-modified folder with the exact contents of the parent folder.

Ideas?

Uncheck both Keep original folder and Move downloads to Destination

This has been resolved, the move downloads to destination button was unchecked, but keep original folder was (hidden). You guys should automatically remove these options if the master feature is unchecked. Save you diagnostics/support in the future.
