Add sqlite3ext.h header #25

It's useful to have extensions when testing new queries with the custom functions in the command line tool.

Yeah, I don't see a reason to omitsqlite3ext.h from the repo. It should probably be also included in release packages, but that's for another commit.

Rebased & merged.
