Sorry for the delay on this. Based on what you said in what's app, it sounds like this is mostly a legal document. Since you know what you want the form to be, If you can do a rough draft, I can polish it, get your feedback, and manage the legal component with the lawyers. (And we agreed we're not doing background checks, correct?)
