Addressed issues with smoking cessation and revised costs #126

Updating master branch with all changes in the final version of my closed_cohort branch, which was used in the published version of the case detection analysis. The only change that was not added to the master branch is described in https://github.com/resplab/epicR/issues/125

So @KateJohnson , case detection no longer affects smoking cessation?

Only indirectly. Smoking cessation occurs at diagnosis regardless of whether or not they've have case detection (but case detection increases diagnosis). If you want to turn smoking cessation off you can set adherence to 0.
