Add diagnosis as a parameter to the smoking change event #93

Diagnosis added as a parameter to the equation for time to smoking cessation (change in smoking status from 1 -> 0).

From the Lung Health Study, smoking cessation increases the quit rate by 17%. The coefficient for diagnosis is ln(1+0.17).

Might also want to add diagnosis to smoking incidence equation. Ie. an (83%) reduction in the rate of starting smoking if diagnosed with COPD. It depends on the smoking intervention being modeled.

This has been upgraded to a separate rate for the effect of smoking cessation. The effect of smoking cessation (OR 1.38 of quitting) lasts for one year after being newly diagnosed with COPD.
