Add forceContinueVoices option for better support of spanners across measures #21

When there are a changing number of voices, spanners don't always work across measures. This pull request adds the option to keep the number of voices constant within a staff.

This adds "noise" to the output code that might not be desired for code that should be manually post-processed, therefore the default remains as-is.

O.K., I'll close this pull request for now as I discovered some problems with meter changes. I might reopen it when I could fix it.
