prepare for apiv1 (new design) #51

The branch is getting larger, because the change is quite dramatic, and even master hasn't stabilized, I propose we merge the active branch into master, and fix CI along the way.

Notable changes include:

use dlmopen for tool DSOs
flesh out echo example
