fix: Multiple tenantId and tp_id conflict

Description

1. Multiple connections with same t_id & tp_id can exist provided they belong to different environment
2. Unique Constraints added uniqueThirdPartyPerTenantPerEnvironment & uniqueThirdPartyPerEnvironment
