address issues and comments in the accuracy section #22

Marcello:

I revised the section on proper accuracy scores. I added several comments in the margin that need to be addressed. This section needs some work. There is a lot of stuff that is not explained so it is difficult to assess what is going on. I get the general idea, but the section needs to be more clearly explained. Also, it is missing a clear formal definition of the proposed accuracy score.

this relates to the older batch of comments which have been addressed. The new batch will be subsumed under #16 .
