Upon booting from a shutdown, the first game launched per-core will play at max volume, and will reset to the previous volume setting after any button is pressed.
This issue seems to happen per-core, launching a genesis game for the first time will cause that game to play at max volume until any button is pressed. If you then exit that game and launch a snes game, that game will also play at max volume until any button is pressed. If you go back and pick a different genesis or snes game, that game will play at the correct volume.
This issue is on Koriki 1.0.1, and the last version I used, 0.9.1, did not have this issue.
